What is the East Side Homeless Network?

Lenox Hill Neighborhood House, Neighborhood Coalition for Shelter and Yorkville Common Pantry, the three leading providers of services to single homeless adults on the Upper East Side and East Harlem, joined together to create the East Side Homeless Network (ESHN). The Network is a collaborative effort to improve and expand access to services for homeless community members. ESHN’s primary goal is to weave services together and break down institutional barriers so that the agencies are better able to bring help to homeless people.

Staff from each agency work together under a structured plan. They share information without client confidentiality and jointly work with the client to develop treatment plans and track progress toward his or her goals. To facilitate a seamless connection between services and providers, ESHN has implemented: an established inter-agency referral protocol; regular joint case conferencing; a protocol on how to share information about clients; and common record keeping and reporting mechanisms.

ESHN clients are eligible for the wide array of services all three agencies offer. Through increased communication about our services, missions, philosophies and goals, we find common ground from which we can work together more effectively. ESHN eliminates duplicate services and ensures that clients do not fall through the cracks. Homeless men and women receive comprehensive assistance to help them move off the streets and, with supportive services, into transitional housing and eventually permanent homes.

Who is the East Side Homeless Network?

Lenox Hill Neighborhood House (LHNH) is a multi-service settlement house that provides assistance to people who live, work or go to school on the Upper East Side. The Neighborhood House’s broad range of services to homeless people includes: a street outreach program; a 24-hour 100 bed shelter for women over 45; an interim housing program; permanent housing and rehabilitation services for formerly homeless, mentally ill adults; follow up case management services for formerly homeless adults who move into independent housing; a full spectrum of vocational services; and consumer-run messenger services.

Neighborhood Coalition for Shelter (NCS) is a coalition of religious institutions, civic groups, and individuals concerned with improving the lives of homeless individuals in its community. Recognizing the interrelatedness of needs, NCS has an integrated continuum of services: the Neighborhood Center for Homeless People, a 24-hour multi-service center for homeless individuals still living on the streets; a 15-bed men’s shelter which provides temporary overnight housing, meals, and rehabilitation programs 365 nights per year to clients of the Neighborhood Center; and the NCS Residence, a 70-unit SRO building that provides safe and supportive permanent housing to formerly homeless individuals. Programs and services, including case management, psychiatric counseling, referrals to medical, educational, and vocational services, and a work experience program, focus on getting the client out into the broader community to gain the confidence and skills needed to live independently.

Yorkville Common Pantry (YCP) is one of the largest meal and Food Pantry programs in New York State. YCP’s mission is to feed hungry individuals and families, and to provide them with the support they need to turn their lives around and become more independent. YCP also operates Project Dignity, a comprehensive program for homeless single adults.
